Health Insurance Plans 

We bill for the insurance(s) with whom we are contracted for both Primary and Secondary Insurances. Specifically, we will submit billing directly to your insurance company. We ask that you provide us with all necessary information to accomplish this. Should you have secondary insurance, we will submit to the primary carrier, and after your primary carrier processes the claim, we will submit the balance to your secondary carrier. We participate in many different PPO and HMO plans and accept traditional indemnity insurance. If you are not sure about our provider status with your insurance company, please check with your insurance company or with our business office during normal office hours. 

Special Note: If you or your family suffers a MVA (Motor Vehicle Accident) many times insurance companies cover all or part of the accident and your health insurance company will cover all or part of the remainder. For those who do not have health coverage we do offer a payment plan for your convenience. There are special circumstances for those who do not have medical payment on their care insurance or have health insurance in which cases you may need legal advice. We work with many attorneys' who would be able to provide assistance in this situation. Please check with us regarding your Insurance Coverage so we may best assist you in this difficult time.

Back to Health Medical Center is committed to providing the highest quality of family medicine at a reasonable fee. We participate in many different PPO and HMO plans and we accept most insurances. Upon visiting our office please remember the following important points with regards to your insurance. 

1. If you do not find your insurance carrier on our "Insurance List" please call the phone number on the back of your insurance card and verify that we are a Select Provider for your insurance. 

2. At the time of service, you will need to bring a copy of ALL insurance cards. 

3. All other insurance carriers with whom we are not contracted with will be billed as CASH. 

4. All Co-Pays are due at the time of service. 

5. We accept Visa and Mastercard.
